When Devices Do the Thinking for RNs, What Training Still Matters?

So I’m sitting at home on a rare day off and I get a phone call. It’s the supervisor trying to locate one of the many devices each staff member has to sign in and out at the beginning and end of each shift. The hospital I work for uses bar code scanners, wireless computers, PDAs, and Vocera badges. These things are supposed to reduce errors and in general make the jobs of staff members easier. Once I get to work I feel like I have to put on a utility belt to carry all of these devices.

With all of these machines to think for me, I wonder if all of the schooling I’m enduring to go from my LPN to RN is obsolete. Yes, compassion and empathy can’t be taught or replaced by technology. But sometimes it seems to me that a technology-savvy teenager could do much of this job, as long as she could stomach the visuals at the bedside. I remember studying night and day for an exam about calculating medication dosages, only to discover that the computers give the exact dosage and that drugs come from the pharmacy just as they should be given.

Kim at Emergiblog has a nice post dealing with changing her mind about whether or not she wanted to get a BSN.

And this post by Anne Dabrow Woods at In the Round (excerpted below) got our attention for its honesty about the difference between treating a condition in the hospital and treating it at home in a family member—and also because it put a human face on an article we ran in our February issue about ostomy complications and management.

My oldest daughter was diagnosed with ulcerative colitis when she was 7 years old and despite aggressive treatment for her disease; she required a total colectomy, temporary ileostomy, and an ileo-anal anastamosis when she was 12. As a nurse I thought I was equipped to care for her ileostomy; was I ever wrong. I had experience taking care of hospitalized patients with ostomies, but I quickly learned caring for someone who is active is a totally different story.

Is the Florence Nightingale Pledge in Need of a Makeover?

Authors and publishers frequently send nursing– and health care–related books to AJN in hopes we will review them. I love it, so keep on sending them. My latest read is Mystery at Marian Manor: The Adventures of Nora Brady, Student Nursea book for young adults. I guess you could call it a Cherry Ames for the new millennium.

At the beginning of the book is the Florence Nightingale Pledge, something I haven’t read since my graduation in 1995. I have to say it made me cringe. It’s almost as bad as when I visit my parents and see the nursing school graduation photo of me in that silly nursing cap I wore under protest. (If the men didn’t have to wear it, why did I?) If you’ve forgotten the pledge, here goes:

I solemnly pledge myself before God and presence of this assembly;
To pass my life in purity and to practice my profession faithfully.
I will abstain from whatever is deleterious and mischievous
and will not take or knowingly administer any harmful drug.
I will do all in my power to maintain and elevate the standard of my profession

Smokers Need Not Apply

Are you a smoker? If so, and you live in Chattanooga, TN, don’t even bother to apply for a job at Memorial Hospital, where being a smoker automatically disqualifies you. To ensure that job applicants are telling the truth, the hospital will subject them to drug tests—for nicotine! This new rule does not apply to current employees, but it does raise some interesting questions. The article notes that such requirements for hire could be a slippery slope on the way to other forms of discrimination. 

As unpopular as this may be, as a fellow health care worker I can see a lot of positives to such a trend. In a previous blog post I discussed nurses as role models for our patients. My focus was on obesity,  but I also mentioned smoking.  

Another recent article, this one in the UK’s Nursing Times, says nursing students in Europe should be encouraged to stop smoking. The article discusses an Italian study reporting that nursing students are twice as likely to be smokers than are members of the general public. Maybe this would be a good policy to promote in the US? The article raises some of the same points I had raised in my earlier post about role modeling and the ways our actions or choices may influence our effectiveness as educators.

Nurses Express Concerns About Colleagues’ Commitment, Training

Back in November AJN clinical editor Christine Moffa posted a short anecdote. She told how she’d been at a training to give H1N1 vaccinations and encountered another nurse with apparent contempt for learning the basic facts about the virus. While it’s obvious that you can’t generalize about the state of an entire profession based on one stranger’s off-the-cuff comment, the responses to this post do indicate that the anecdote touched a nerve in some readers and that other nurses have also had similar experiences with their colleagues. Here are some brief excerpts from longer comments:

From Naomi: “If I fail a class by 1 point and go to the director of my nursing program demonstrating my professionalism, critical thinking, and self responsibility i would get a pat on the back and a registrar’s form in the hopes that I could repeat the class if there are enough seats. My 3.8 GPA allowed me gain admission into my nursing program not my professionalism, critical thinking, and self-responsibility even though those are key qualities for a good nurse.”

From Nursevon: “I am a faculty member in an undergraduate BSN program at a university in the midwest. I have become increasingly discouraged in my job as an educator. The focus of students is very short-sighted: typically on points and grades. As hard as I try to instill professionalism, critical thinking, and self-responsibility for one’s own learning, I frequently come away profoundly discouraged.”
