A Nurse’s Lessons from Hiking the Appalachian Trail

Burnt-out and realizing it’s time for a change.

I had always been able to juggle family, school, and work life well, including roles as class mom, scout leader, and community volunteer, completing my doctorate in nursing, and working two jobs. But at a certain point, and despite my supportive family’s efforts, I began to burn out. Working as a nurse leader for a behavioral health unit was a dream come true and a nightmare all in one. I found myself caring so much, investing so much of myself, that I lost myself in the process.

In the hope that it would help, I moved back to nursing education. It didn’t. There was a void. I was missing something. I was missing me.

Maybe this is what burnout on the way to compassion fatigue feels like. But whatever we call it, my response was to quit my job and hike the Appalachian Trail for five months from Maine to Georgia with my husband. We’d always lived simply, and once we’d made the decision to go, the pieces fell into place.

Along the way, I made many discoveries. It’s paradoxical that I’d gone hiking to forget about nursing, yet I was reminded about it with each step.

Caring for Caregivers—We Need More Than Self-Care and Resilience

Self-care is not a panacea.

Since the start of the pandemic, AJN has received many manuscripts and queries related to self-care and resilience to prevent burnout. It’s not surprising, given that this has been a harrowing year for nurses.

But while self-care and resilience are important, and such articles are needed, all the self-care in the world can’t fully address the root of the problem—the systemic issues that lead to burnout. At some point health care administration needs to step in and become part of the solution and offer staff the help they need.

Nurses: In the Face-Off With COVID, We’re Doing ‘Alright’

We did alright during the HIV/AIDS epidemic. We’ve done alright during Ebola. We’re doing alright during COVID. We’ve done alright through war, earthquakes, tsunamis, landslides, and typhoons. We’re neither politicians, nor business owners; we’re not in banking or finance (though some of us might have a side hustle or two).

We’re nurses, and we show up.

Let’s be clear, alright is not “OK,” it’s not “good,” and currently it’s “head barely above water.” Alright is persistence, though, and it’s perseverance. We’re all suffering. Those of us who have worked in past epidemics are having flashbacks, we still don’t have enough protective gear, and we’re scared to death of bringing COVID home to our loved ones.

Here’s a real gem, to boot: When COVID started and picked up exponential speed in the U.S., we were touted as heroes. And now as the waves of cases roll in again, because of the extraordinary financial hit that health care has taken we’re the first to be flexed off and furloughed.

Can We Ever Overcome Burnout in Nursing?

It seems to me that we’ve been talking about burnout about as long as I’ve been in nursing, and that’s over 40 years. In 1974, Marlene Kramer’s book Reality Shock: Why Nurses Leave Nursing reported on how nurses’ dissatisfaction with their inability to practice as they were taught was a major factor in their leaving the profession. (Here’s AJN’s 1975 review of the book.) In the 1980s, it was the downsizing of staff that caused many to leave (see the February editorial for my own experience). In the last decade, as health system changes and staffing (again) engendered moral distress and burnout among members, nursing organizations sought ways to mitigate distress among nurses.

But the issue persists and arguably has gotten worse, with increasingly alarming reports of high levels of burnout—between 34% and 54% physicians and nurses report symptoms—and suicides.
