Understanding Imposter Syndrome Among Nurses

The self-doubt began when I applied for nursing school. I’d think, “What makes you think you should have this sort of responsibility?” or “I can’t possibly do this— everyone says nursing school is so hard.” I’d been accepted into two different programs, but this only heightened my suspicion that there had been some mistake. I worried that once I was in school people would realize that I didn’t belong there.

These beliefs were hard to shake. I thought I was the only one with such doubts, but it turns out that I’m not alone and this phenomenon has a name. Imposter syndrome (IS) is the feeling of being a fraud. It often affects women and minorities, and it tends to occur at times of career transitions and in high-achieving individuals. Nurses can be particularly affected.

What is it?

In 1978, Pauline Rose Clance and Suzanne Imes published their study on what they called the imposter phenomenon. Now commonly called imposter syndrome, it is described as having deep feelings of unworthiness and doubt of one’s capabilities, coupled with the fear of being “found out” or exposed as a fraud. Learning on the Fly: Thoughts on Birding and Nursing During a Pandemic

It’s my opinion that every nursing textbook should open with the above statement. However, it’s from the introduction to Sibley’s Birding Basics, by David Allen Sibley.

During home isolation of the COVID-19 pandemic, my husband and I took up birdwatching as a way to get out of our home and entertain ourselves while maintaining our “bubble.” Armed with binoculars, David’s camera, and my artist’s field bag, we visit local wetlands and wildlife reserves, recording our finds. That led me to read Birding Basics.

Experience coupled with pattern recognition.

As a nurse, I can’t help but link the idea of “interpreting what you see and hear in order to make better judgments” as a definition of a nurse’s intuition, commonly referred to as a “nurse’s gut.”

What Would It Have Helped to Know as a New Nurse?

An incomplete record of starting out as a nurse. 

When I think back on my first year as a nurse, I always say two things to myself: “I wish I had written more,” and “I wish someone had given me a more realistic how-to manual.” I try to remember patients from back then. What would I have shared, had I written about each one?

I was never not writing about something (sometimes what I wrote was published on this blog), but over time, as I moved away from bedside care and into administration, I wrote differently: policies and program plans, research protocols and systematic reviews. Although I rarely worked directly with patients anymore, their positive outcomes continued to motivate my every word.

I like to think my writing has grown with me. I’ve learned the power of the active voice; the structure required for the APA Writing Manual. 7th edition; and the deepening of understanding that comes from reading and reflection. Welcome to Nursing’s New Grads!

Nervous and excited is normal.

It’s late June and by now most new grads are beginning their careers. Based on tradition, most of you will be working in the acute care setting. Some may have found positions, like my niece did when she graduated, in out-of-hospital surgical centers, or perhaps you’ve taken a position in a nursing home or long-term care facility. Wherever you’ve landed, we know you’re likely to be a mix of nervous and excited.

First, don’t worry—everyone, even the nurse manager who scares you, was new once. Over time, you’ll get more comfortable with your skills and gain confidence.

The basics.

Be prepared, show up on time, ask questions; be the person you’d like to work with. Offer to help others when you can and you’ll find it easier to ask when you need a hand.
